Mission Statement

The mission of the Apple Continuum of Care program is to ensure safe, uninterrupted access to methadone and other Medications for Opioid Use Disorder (MOUD) for individuals transitioning into skilled nursing care. We are dedicated to providing compassionate coordination—bridging hospitals, opioid treatment programs, and Apple Rehab facilities—to promote recovery, stability, and dignity at every step of the healing process.
